Which forms and templates do I need?
Signature and Handwriting Credentials form
Complete this form as part of the Florence onboarding process. It records your printed name, signature, initials, handwritten numbers and date format, helping study teams identify handwritten documentation held outside Florence in source data.
The completed form should be stored centrally as a certified copy at organisational level and linked to individual studies when required.
Florence Attestation
The Florence Attestation is one of the mandatory records users must upload to the appropriate folder in the Training and Education binder after receiving access.
Florence SOP
The webpage specification identifies a Florence SOP as a future resource, but the SOP was not included among the supplied documents.
How should I name items in Florence?
Consistent naming keeps Florence searchable, supports accurate reporting and makes it easier to retrieve records across studies, locations and functions.
| Item | Naming format and example |
|---|---|
| Files | YYYY-MM-DD + file name or type + version. Example: 2026-08-12 Protocol v1 |
| Staff names | First name + last name. Do not use titles or initials. |
| Logs and templates | Lead department + log type + version or date. Example: Oncology delegation log v1 |
| Roles | Department + blinded status + role type + study name |
| Binders | Lead department + IRAS number + blinded status + study name. Example: Oncology 358442 OL SCUMBLE study |
What blinded-status abbreviation should I use?
Use:
- DB for double-blind studies where all Barts Health staff are blinded
- SB for single-blind studies where different Barts Health team members may be blinded or unblinded
- OL for open-label studies with no blinding

Who can create roles and binders?
Only local Florence Champions and Deputy Champions can create binders or create and manage study-specific roles.
Do not add a number to the beginning of a role name. This ensures global template roles remain at the top of the roles list.
